It would not be sufficient to make plans every six months. After all, a client’s life changes all the time. They may have one interest one day, and another the next.
For example: a client may be introduced to a hobby by a friend or someone they have recently met, and want to participate in it on a regular basis, or they become ill and their immediate needs change. In such circumstances they will need some new plans to be made.
The client’s Key Worker will develop new goals with them, and where necessary other health and social care professionals, and collaboratively develop appropriate plans to achieve them, as the issues arise.
These goals will be written on to their personal goal plan. The plan will guide others how the client’s goal (the thing they want to do), is to be carried out, and what items are necessary in order for it to be done successfully.
